1. Overview: What this path actually is today
For most of the period from 2022 to 2025, the UK Health and Care Worker Visa allowed people from outside the UK to enter the country as care assistants and support workers. The demand was real, the UK has an ageing population and a severe shortage of care staff. Hundreds of thousands of international workers from Nigeria, Zimbabwe, the Philippines, and across Africa used this route.
That route is now closed.
On 22 July 2025, as part of a major overhaul of the UK Skilled Worker immigration rules, the UK government officially closed the overseas care worker visa route to new applicants. Entry-level and senior care worker roles (SOC codes 6135 and 6136) can no longer sponsor new workers from outside the UK.
This does not mean there are no paths into UK healthcare. It means the path has changed and this guide explains both what happened and what the real options are now.
2. Eligibility: What the rules currently say
Who is fully blocked (as of 22 July 2025)
Anyone outside the UK applying for a new care assistant or senior care worker role under any visa category. The minimum skill level for Skilled Worker visa sponsorship has been raised from RQF Level 3 to RQF Level 6 (degree level). Care assistant roles sit at RQF Level 2–3, which is below the new threshold.
Who still has options inside the UK
If you are already living and working in the UK as a care worker with an existing Health and Care Worker or Skilled Worker visa in SOC 6135 or 6136 (granted before 22 July 2025), you can:
Extend your visa with the same employer until 22 July 2028, provided you meet the salary and compliance requirements.
Switch to a new employer in the same SOC code under transitional provisions - but only if continuous permission is maintained.
Critical note on dependants: If your first care worker visa was granted on or after 11 March 2024, you cannot bring family members (dependants) to the UK under this route.
Who can still enter the UK from overseas in healthcare
The Health and Care Worker Visa is still fully open to:
Registered Nurses (NMC-registered, any specialty)
Doctors and dentists (GMC-registered)
Occupational therapists, physiotherapists, radiographers, and other allied health professionals
Other clinical and specialist roles at RQF Level 6 and above
This is the path that now matters for people in Africa who want to work in UK healthcare.
3. Skills employers actually want
This section covers registered nurses, since that is the realistic pathway for overseas applicants.
Based on analysis of NHS job postings and Trust recruitment requirements in 2025–2026:
NMC registration - mandatory; no NHS Trust will hire you without it
English language proficiency — IELTS Academic score of 7.0 overall (minimum 6.5 in writing), or OET with three grade Bs and a C+ in writing
Active registration in your home country - you must currently be registered as a nurse with your home nursing council
Clinical hours and degree equivalency - a nursing diploma or degree equivalent to UK RQF Level 5, with at least 3 years of study including 500+ hours of theory and 500+ hours of supervised clinical practice. Most nursing degrees from Nigeria, Ghana, Kenya, Zimbabwe, and the Philippines meet this standard.
Soft skills - patient communication, documentation accuracy, ability to work in multicultural NHS teams, adaptability to UK clinical protocols
High-demand nursing specialties as of 2026: theatre nursing, emergency nursing, mental health nursing, oncology, diagnostic radiography.
4. Step-by-step path: From overseas to NHS nurse in the UK
This is the realistic route in 2026. It takes 12–24 months end to end and is based on synthesised data from the NMC, NHS Trust recruitment programmes, and real applicant timelines.
Step 1 - Confirm your nursing qualification is eligible Your nursing degree must be equivalent to UK RQF Level 5 and your training programme must have included at least 500 hours of theory and 500 hours of supervised clinical practice over a minimum of 3 years. Most degrees from Nigerian, Ghanaian, Kenyan, and South African nursing schools meet this standard. The NMC assesses each application individually; there is no pre-approved country list.
Step 2 - Pass an English language test Sit IELTS Academic (minimum 7.0 overall, no component below 6.5 except writing at 6.5) or OET (minimum three Bs and a C+ in writing). Book early test slots fill up fast and results take 3–5 weeks. Your result is valid for 2 years.
Step 3 - Apply to the NMC and pass the Computer-Based Test (CBT) Submit your application to the Nursing and Midwifery Council (nmc.org.uk) with your qualification, home country registration, and English test result. Once your documents are verified, you will be invited to sit the CBT - a 120-question multiple-choice test of nursing knowledge. It is available at Pearson VUE test centres in Nigeria (Lagos, Abuja, Port Harcourt), Ghana, Kenya, and many other countries. The fee is £83. CBT results are usually available within 48 hours. Your CBT pass is valid for 2 years you must book your OSCE within this window.
Step 4 - Secure a job offer from an NHS Trust and get a Certificate of Sponsorship This is the step most people try to skip - don't. Apply to NHS Trusts directly via jobs.nhs.uk or through a reputable recruitment agency (see Section 6). Most Trusts want to see your CBT pass before making an offer. Many NHS Trusts cover the OSCE cost as part of their offer, and some cover visa fees and relocation too. The Trust issues a Certificate of Sponsorship once the offer is made.
Step 5 - Apply for the Health and Care Worker Visa With the Certificate of Sponsorship in hand, apply for the visa online. The Health and Care Worker Visa typically takes 8 weeks to process. Fees as of 2026: £284–£551 (discounted from the standard Skilled Worker rate). Once approved, you will enter the UK on a visa linked to your sponsoring NHS Trust.
Step 6 - Travel to the UK and sit the OSCE The Objective Structured Clinical Examination (OSCE) must be sat in the UK. It consists of 10 clinical stations simulating real nursing scenarios. The current pass rate is approximately 90%. Your sponsoring NHS Trust will typically arrange this. Once you pass, the NMC issues your PIN (registration number), and you can legally practise as a nurse.
Step 7 - Begin work and build your career You start as a Band 5 Nurse (£31,049–£37,796/year in England, 2026/27). After 5+ years you progress to the top of Band 5 automatically. Specialist training opens Band 6 roles (£38,682–£46,580). After 5 years of lawful UK residence you may be eligible for Indefinite Leave to Remain (ILR/settlement).
5. Real-world challenges
These come from BBC investigations, immigration lawyers, and reports from applicants and former care workers.
Scams are massive and still active. The UK care worker visa attracted a large organised fraud industry agencies in Nigeria and elsewhere charged applicants £5,000–£13,000 for certificates of sponsorship for jobs that did not exist. One BBC investigation found a single operator sold over 1,000 fake placements. The UK government revoked more than 470 care sector licences between 2022 and 2024, affecting 39,000+ workers. Even now that the direct care route is closed, scammers continue to advertise "UK care jobs with visa sponsorship" to unsuspecting applicants. If anyone asks you to pay to get a Certificate of Sponsorship or a job offer, it is a scam.
Legitimate recruitment is always free. Under UK law, employers and agencies cannot charge candidates for job placements. The employer pays all sponsorship fees. If you are being asked for money upfront, stop immediately.
The OSCE must be sat in the UK. This is a genuine cost and complexity you need to travel to the UK while your visa application is in progress or after it is approved. Most NHS Trusts handle this as part of their onboarding.
NMC processing times are unpredictable. The NMC has faced backlogs. The initial document review stage can take 3–6 months. Plan accordingly.
The July 2025 closure devastated many applicants. Reports from Zimbabwe, Nigeria, and Ghana describe people who had resigned from jobs and were weeks away from their sponsorship being processed when the route closed. If you are in this position and you are already in the UK on an existing care worker visa, you may still have options speak with a regulated UK immigration adviser (OISC Level 2 or above).

8. Mistakes to avoid
Paying anyone for a job or sponsorship. This is always a scam. The employer or NHS Trust pays all costs. You never pay for a Certificate of Sponsorship.
Trusting agents advertising "UK care assistant visa sponsorship 2026." The direct care assistant route is closed. Any agent saying otherwise is either uninformed or running a scam.
Applying without completing your CBT first. Most NHS Trusts require at least a CBT pass and sometimes an NMC conditional offer before they will make a job offer. Don't send generic applications without this in place.
Ignoring the English language requirement. The NMC will reject your application if your IELTS or OET score does not meet the threshold. Take the test seriously and prepare for it. The writing component is where most international applicants fall short.
Waiting passively. NHS Trusts hire in cohorts. If you are in the pipeline, follow up actively. Register with multiple Trusts and agencies simultaneously.
Using unregulated immigration advisers. In the UK, only OISC-regulated advisers and practising solicitors are legally allowed to give immigration advice. Using an unregulated agent puts you at risk of bad advice and potential fraud.
